Tinnitus Spikes: Why They Happen and How to Handle Them

You were making progress. The ringing had settled to a background hum you barely noticed. Then a busy week hit — a stressful deadline, a bad night's sleep, a glass of wine with dinner — and suddenly the noise is back, louder than it's been in weeks.

This is a tinnitus spike, and if you've experienced one, you know the particular dread that comes with it. Not just the sound itself, but the fear that the progress you've made is gone, that you're back at the beginning.

You're not. Here is what's actually happening, and what to do about it.

What a spike actually is

A tinnitus spike is a temporary increase in the perceived loudness or intrusiveness of your tinnitus. "Temporary" is the operative word. Spikes are not relapses. They are not signs that your tinnitus is permanently worse. They are a normal and almost universal feature of tinnitus management — something that happens to people who are doing well with their symptoms, not just people who are struggling.

The distinction matters because how you respond to a spike shapes how quickly it resolves. Treating it as a catastrophe triggers the stress response, which prolongs it. Understanding it as a predictable fluctuation allows you to manage it calmly, which shortens it.

Why spikes happen

Tinnitus perception is not fixed. It's a signal generated by your brain's auditory system, and that system is sensitive to your body's overall state. Several factors reliably tip the balance toward a louder signal.

Stress. The most consistent trigger. Stress hormones, particularly cortisol and adrenaline, increase neural excitability across the brain, including in the auditory cortex. Your tinnitus doesn't change, but your brain's sensitivity to it does. The feedback loop can be vicious: the spike causes more stress, which sustains the spike.

Poor sleep. Sleep deprivation lowers the threshold for almost every unpleasant sensory experience. After a bad night, pain feels sharper, light feels brighter, and tinnitus feels louder. One poor night can produce a noticeable spike; a run of them can sustain one for days.

Loud noise exposure. A concert, a power tool without hearing protection, even a particularly loud restaurant — noise exposure can trigger a temporary worsening that lasts hours or days. The cochlea and the auditory nerve are briefly overloaded, and the brain compensates by increasing its internal gain.

Caffeine and alcohol. Both affect the vascular and neurological systems in ways that can temporarily amplify tinnitus in some people. Caffeine is a stimulant and can increase neural excitability. Alcohol, paradoxically, tends to disrupt sleep architecture — you fall asleep faster but spend less time in restorative deep sleep, leading to the same fatigue-driven spike you'd get from a restless night.

Illness. Ear infections, sinus congestion, colds, and general systemic illness often cause temporary spikes. The middle ear is sensitive to changes in fluid pressure or inflammation; the auditory nerve is affected by the same immune responses that make you feel unwell.

Medications. Several common drugs — aspirin at high doses, some antibiotics, certain diuretics, and NSAIDs — are known to cause temporary tinnitus worsening. If a spike follows a new or changed medication, note it, and mention it to your doctor if it persists.

What to do during a spike

The most important rule is one that's easy to state and hard to follow: do not try to carefully assess your tinnitus during a spike.

The impulse to monitor — to listen closely, to compare it to yesterday, to search for signs it's improving — is understandable but counterproductive. Every deliberate check sends your brain a signal that the tinnitus is worth close attention. That attention is precisely what habituation training is working to undo.

Stick to your routine. Use your sound therapy as normal. If anything, a spike is the moment when consistent sound therapy matters most. A steady background sound competes with the elevated signal and prevents the silence-amplification effect from adding further layers to an already difficult moment.

Identify the likely trigger. Not to blame yourself, but because understanding the cause tells you what to expect. A stress spike tied to a single hard day might clear overnight. A spike after loud noise exposure often takes two to three days. An illness-related spike typically resolves as you recover. Knowing the cause reduces the uncertainty that amplifies anxiety around the spike.

Manage what you can. If stress triggered it, address the stress directly — sleep, gentle movement, breathing exercises, time away from whatever is creating the pressure. If sleep is the issue, protect it. If the spike is tied to a lifestyle factor you can control, act on that factor rather than waiting passively.

Don't withdraw. A common response to spikes is to cancel plans and stay home to protect yourself from noise and stimulation. This tends to make things worse. Reduced activity increases rumination; social withdrawal increases anxiety. Keep living normally while the spike runs its course.

Give it time. Most spikes resolve within a few days to two weeks without any intervention beyond the above. If a spike persists beyond two weeks without an obvious ongoing cause — illness, sustained stress, continued noise exposure — it is worth checking in with your audiologist or ENT.

What spikes mean for your progress

Spikes do not erase progress.

Habituation is a change in how your brain processes and classifies tinnitus, not a change in how loud your tinnitus is. When a spike occurs, your auditory system is temporarily more reactive, but the neurological learning you've accumulated remains. When the spike resolves — and it will — you return to the level of habituation you had achieved, or very close to it. People further into their tinnitus management journey often find that spikes resolve faster than they did at the beginning, because the underlying habituation is more established.

The worst outcome of a spike isn't the spike itself. It's deciding, in the middle of the worst days, that the approach isn't working and stopping. That decision costs you the progress you've already built.
